1) Amidala has children by Anakin, but he does not necessarily know about them. He definitely does not know about Leia, and he probably doesn't know about Luke, either. First of all, they're twins, so it'd be hard to know about one and not the other. Secondly, judging by the importance placed on finding the Son of Skywalker in ESB, a manhunt would be underway for Luke from the beginning if Anakin did know. This does not seem to be the case.
He may know that she is pregnant, but he is probably under the impression that she is dead by the end of EIII, along with her unborn child.

4) Yoda escapes to Dagobah, yes. Judging by the dark cave, this will probably be featured in EIII, so as to explain it. The cave probably has Sith affiliations, but it need not be the reason Yoda chooses the location. We shall see what the deal is there.

Any which way you cut it, EIII will be packed with different plot threads. I'm looking forward to it!
So, plot points EIII will certainly cover (as much as anything can be certain):
-Jedi Purge. This will probably be introduced near the beginning of the film, as it is an important plot thread, and it needs to take place over a duration. Anakin probably will not initially be involved, but will join it when he splits from Obi-Wan.
-Anakin and Obi-Wan part ways. This will probably involve him leaving the Jedi Order behind.
-In leaving the Order, I expect a final good bye between Anakin and Amidala. They will have realized that their visions are not compatible, and they will part as opponents, if not enemies. Something of a divorce...they may still 'love' each other, and they're definitely sad that it didn't turn out better...but there's no escaping the seperation. Refer to the TPM novelization for a young Anakin's dreams of an older, sadder Padme leading armies against him. Don't have the book handy, but it's his dream the morning of the Boonta Classic, I believe.
-The Clone Wars must come to a conclusion. This means that the Confederacy will need to be defeated in combat. This is no small task to portray, I expect the combat to take place for the duration of the film.
-Mace Windu will die. It has been promised to be a death to remember. Mr. Wong has voiced his opinion of just how memorable it should be ;)
-Yoda and Count Dooku will duel. Dooku to Yoda in AOTC: "This is only the beginning!"
-The mother of all duels will take place between Obi-Wan and Anakin. It has been noted that Obi-Wan will win, and indeed, it seems so, but remember that Anakin will leave it thinking that he won. The only thing we know for certain about the outcome is that Anakin will be horribly disfigured, Obi-Wan will not, and the galaxy will think that Obi-Wan is dead.
-The Jedi Purge will come to a close, or will at least be conveyed as having been successful. That's a lot of dead Jedi. *sniff* poor little Bear Clan.
-Palpatine will find himself in the proper political climate to be able to proclaim himself Emperor.
-The founding of the Alliance will probably be portrayed. Bail Organa will be involved, and perhaps a young Mon Mothma as well.
-The clone troopers will evolve to stormtroopers. This change will be explained.
-Luke and Leia will be spirited away to their respective adoptive families. I presume Obi-Wan will take Luke to Tatooine, and Amidala herself will probably take Leia to Alderaan. Representative Binks may or may not accompany Amidala.
-anything else?
Other events that may take place, but are not certain:
-Possible duel between Anakin and Dooku? Would fit, but that may be too much to ask for in one movie.
